Overview

Join Linda Garrett and Andrew Garrett of Garrett Consulting as they present:

• new rules expanding hospitals' duties to develop written discharge policies and processes for all patients;

• the need for discharge plans to include appropriate arrangements for post-hospital care, including notification and counseling for caregivers, as needed;

• the additional services and resources that must be offered to homeless patients at discharge, including for example, weather-appropriate clothing, a meal, a prescription or medications, and transportation within 30 minutes or 30 miles;

• the need for planning to include collaboration with other community agencies and organizations that can provide homeless patients with treatment, shelter, and other supportive services; and,

• the patient confidentiality issues and concerns that must be addressed when engaging in the new multi-agency approach to discharge planning.
