Cal/OSHA CCR 3380, Personal Protective Devices, requires that employers assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of Personal Protective Equipment (PPE).  Agencies must document this process through a written certification.

The supplementary Sample Form is intended to be used for the required written certification. On it, employers document the determination if the hazard(s) can/cannot be eliminated without the use of PPE. If the answer is ‘no,’ then the required PPE is identified. The form includes eye and face, head, hand, foot, leg, body, fall, respiratory, and hearing protection.
